FACES for Parents Informed Consent and Waivers Community and Personal Safety Agreement It is essential for the safety of all concerned, and for the credibility and reputation of the FACES program, that everyone is clear about the standards of personal conduct while participating in FACES programs. Adherence to the following standards helps create safety for all and maintains an environment of learning and growth. By checking each box below you are committing to living these standards of conduct while participating in FACES programming. I agree to focus on creating healthy, and respectful relationships with other group members. I understand that recreational drug use, and consumption of alcohol are all prohibited while participating in FACES programming. I understand that smoking, vaping, and tobacco use are only permitted during breaks and in designated areas. I understand that I am in important part of the risk management plan at FACES and will do my part to keep myself and others safe; this includes following directions, utilizing safety equipment as outlined, and looking out for others. General Safety * There are inherent risks with participating in activities, including the kind of travel, accommodation and indoor and outdoor activities provided by FACES Education Inc. Our program and activities are run by a variety of leaders and guides with various training in group management, risk management, and activity-specific skills. We seek to follow industry safety-standards within our various activities and seek to create an experience that puts participant safety as a primary priority. We are proud of our safety record and invite you to inquire of us about our safety record. Examples include: Criminal-record checks and background checks on staff; Staff in each session certified in general group safety and risk management in an outdoor setting; Staff in each session who are certified in Wilderness First Aid (20-hour or 40-hour); Climb-sites set up and supervised by ACMG top rope instructors; Flat-water (lake) canoeing instructed by Outdoor Council of Canada instructors; Hiking guided on established trails by Outdoor Council of Canada certified leaders who are experienced and familiar with the specific trails we are utilizing. I understand that the likelihood of a safe experience is increased when I listen to instructions and follow them; I understand that I'm increasing the likelihood of FACES leaders responding to my needs by fully disclosing pre-existing medical conditions, health concerns, injuries, or any such medical needs that arise while participating in FACES programming. I understand that I must use the safety equipment and gear as directed. I understand that that staying in groups of at least four is standard practice in attempting to reduce the risk of wildlife encounters. I understand that it is my duty to report safety concerns to a FACES representative. I understand that it is my duty to look out for my own safety as well as the safety of those around me. I understand that EVEN AFTER ADDRESSING SAFETY, TRAVEL,TRAINING AND ACTIVITY-SPECIFIC ISSUES THERE REMAIN INHERENT RISKS ASSOCIATED WITH ACTIVITIES UNDERTAKEN BY OR FOR FACES THAT ARE ASSUMED BY ME. Understanding the Risks * When you arrive at the FACES for Parents venue, you will be asked to confirm that you understand the risks inherent in FACES activities, that you assume responsibility for these risks, and that you have waived your right to sue. I am aware of the risks associated with or related to outdoor and adventure-based activities (including the risk of severe or fatal injury to myself, or others). I am aware that I am subjecting myself to the risk of all manner of muscular injuries, bruises, scrapes, cuts, sprains, dislocations, broken bones; head, facial or dental injuries; I am aware that any of these injuries might result from an accident while hiking, climbing, rappelling, walking, playing games, swimming or canoeing, including the risk of drowning; I am aware that I am subjecting myself to the risk of injuries or death resulting from travel by motor vehicle to and from pick-up points, and to activity venues used during the FACES program; I am aware that I am subjecting myself to the risks associated with weather or temperatures and that these may result in heat-stroke, sunstroke, or hypothermia; I am aware that I am subjecting myself to the risk of any manner of injury or illness from curious or aggressive wildlife, including but not limited to bears, wolves, and other carnivores, snakes, ticks, other insects, parasites and other domestic and wild animals; I am aware that I am subjecting myself to the risk of minor scrapes, sprains, bruises and campfire burns resulting from outdoor camping activities in general and that infection may follow such injuries; I understand that I am subjecting myself to the risks associated with being in remote locations with poor communication & the inability to get rescue or medical assistance quickly or easily; I am aware that I am subjecting myself to the risks associated with cuts, abrasion, rope burns and other injuries resulting from indoor or outdoor rock climbing or rappelling; and other injuries resulting from activities such as climbing, belaying, rappelling, rescue systems and any other rope techniques; I am aware that I am subjecting myself to the risk of incurring injuries resulting from failure of ropes, slings, harnesses, climbing hardware, anchor points and rock structures, including falling rocks; I am aware that I am subjecting myself to the risk of death, injuries or illness resulting from failure to follow directions from instructors or those in charge of outdoor trips; I am aware that I am subjecting myself to the risk of intense emotional responses and interpersonal discussions; I am aware that I am subjecting myself to the risk of triggered responses and memories, reliving trauma or hearing other's trauma; I am aware that I am subjecting myself to the risk of medical problems arising before, during or after the trip; and any injuries, illness or death resulting from unexpected events, disasters, or weather. Waiver of Rights * By checking the following you are waiving your right to sue I understand that I AM WAIVING CERTIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE. I am aware of the risks associated with or related to outdoor and adventure-based activities (including the risk of severe or fatal injury to myself, or others). I agree to this Assumption of Risk, Release of Liability, Waiver of Claims, and Indemnity Agreement. I understand that I am releasing FACES Education Inc, and their directors, officers, employees, and representatives from any liability. I understand that I am waiving my rights to sue. Video and Photography * I understand that while participating in FACES programming I may be recorded in video, photos, and audio recordings. Videos, photos, and audio containing my likeness and voice may be shared with others, posted to social media, and used for marketing and promotional purposes.
